What you can receive

General Physician, P.C. settlement payments are between $60 and $5,000 per person, depending on which payment option you choose. Class members can elect Cash Payment A, which reimburses documented out-of-pocket losses related to the breach up to $5,000, or Cash Payment B, a flat estimated payment of $60 that may adjust up or down based on how many valid claims are filed. In addition to either cash payment, you can also claim two years of Credit and Medical Records Monitoring through CyEx Medical Shield, valued at an estimated $179.40 per year. Payments will be distributed after the court grants final approval of the settlement and any appeals are resolved.
